WHAT FORMS DO I NEED?

 

The forms that you will need are specific to your individual project. Please visit the International Science and Engineering Fair website and complete the "Rules Wizard" to determine what forms you will need.

Doing the necessary forms is often a confusing time. But it's not that bad. In fact, if you follow the chart, you'll see that most of you only need to do ONE form!!

After doing the WORKSHEET page, you should be able to figure out what forms you need to do before you start your experiment.


IF YOUR PROJECT HAS:
A.) NO HUMANS, NO VERTEBRATES, NO POSSIBLY HARMFUL CHEMICALS, NO POSSIBLY HARMFUL ORGANISMS.   
THEN THE FORMS NEEDED ARE:
A.) RESEARCH PLAN AND APPROVAL FORM ONLY

IF YOUR PROJECT HAS:
B.) HUMANS INVOLVED IN A SURVEY  
THEN THE FORMS NEEDED ARE:
B.) RESEARCH PLAN,  HUMAN SUBJECTS FORM,  INFORMED CONSENT FORM FROM EACH PERSON PARTICIPATING AS A RESEARCH SUBJECT
 
IF YOUR PROJECT HAS:
D.) VERTEBRATE NON-HUMAN ANIMALS
THEN THE FORMS NEEDED ARE:
 D.) RESEARCH PLAN AND APPROVAL FORM, QUALIFIED SCIENTIST FORM, AND VERTEBRATE ANIMAL FORM.

IF YOUR PROJECT HAS:
E.) HARMFUL CHEMICALS
THEN THE FORMS NEEDED ARE:
 E.) RESEARCH PLAN AND APPROVAL FORM, QUALIFIED SCIENTIST FORM.


 IF YOUR PROJECT HAS:
F.) HARMFUL ORGANISMS (LIKE DISEASE-CAUSING BACTERIA, POISONOUS PLANTS, ETC.)
 
THEN THE FORMS NEEDED ARE:
F.) RESEARCH PLAN AND APPROVAL FORM, AND QUALIFIED SCIENTIST FORM


    
Now, each project is different, so be sure to check with your teacher or the science fair director at your school to make sure you do the forms you need to do.  If you think your project is particularily complex or difficult, make sure you ask your teacher what forms you need to be doing.
